Seeking a Full-time Transition Specialist to provide transition assistance to individuals on an interdisciplinary team serving 68 adults diagnosed with serious mental illness. Position involves significant fieldwork and home visits. The Transition Specialist will assess individuals for transition readiness, provide linkage and escorts to appropriate outpatient providers as well as provide post discharge follow-up. Services are provided both individually and in group settings. Must be detail oriented, have excellent communication skills and be able to document in an electronic health record. Preferred candidate has a Bachelor’s Degree and have at least one year of experience in a comparable position as well as a driver’s license with a clean driving record.
