Assertive Community Treatment (ACT)

The Bronx, Brooklyn and Queens ACT Programs provide ongoing, community-based behavioral health services to those with histories of mental illness, repeated inpatient admissions, and difficulties in obtaining and/or maintaining treatment in outpatient settings. 

 

Personalized recovery oriented services (PROS)

The PROS Program includes an array of comprehensive rehabilitation services for adults with serious mental illness. PROS enables participants to pursue life goals with a focus on four domains: living/residential, education, socialization and employment. The program structure provides participants with the education and clinical support needed to promote improved well-being and independence by developing skills and abilities necessary to overcome barriers caused by mental illness.

The program offers groups in all evidenced based practice areas, including clinical services, Treatment for co-occurring disorders and family psycho-education through collateral visits.

WHAT WE OFFER

Community Rehabilitation and Support services are the main service component of PROS. Designed to engage and assist participants in developing the skills and supports necessary to live as independently as possible in the community.

Intensive Rehabilitation services are designed to assist in attaining specific life roles. These services can also help to reduce the risk of hospitalization, loss of housing or involvement with the crim-inal justice system and allows an opportunity for family involvement in supporting and/or understanding the recovery experience.

Ongoing Rehabilitation and Support services are designed to address the needs as they join and maintain a position in the competitive workplace. This includes providing the ongoing vocational counseling, advocacy & support while on the job.

 

 

Care and Case Management Services (Health Home)

PCMH actively participates in providing care coordination services to both children and adults with behavioral health and/or chronic medical conditions in a variety of community settings.
