Seeking an Outreach Specialist to conduct outreach and enrollment services through telephone, written, and face-to-face contact with eligible individuals. The Outreach Specialist is responsible for identifying, locating, engaging and enrolling individuals who have been referred for Care Coordination services. Outreach and engagement activities may also include education on managed health care options and public health programs to individuals at provider sites and/or the individual’s residence. Significant field work involved. High School diploma or GED is required; CASAC, Associates, or BA preferred. MUST have 1-2 years’ experience working with individuals with chronic mental health, substance abuse or medical issues.
