Residential Services
PCMH operates a residential continuum of care comprised of four community residences as well as congregate and scattered site supported housing. A major developer of housing for persons with mental illness, PCMH owns four apartment buildings in the city and is expanding its commitment to reduce homelessness among New York City's most vulnerable citizens. PCMH's community residence programs assist consumers with the difficult transition from long-term or revolving-door hospital stays to life in the community. When a consumer has developed the stability and skills needed to live independently, they may move to a supported housing apartment operated by PCMH. A more independent level of housing than community residences, tenants pay a portion of their income towards their rent and the balance is made up through a rent subsidy. Consumers may stay in the supported housing program for as long as they wish.
Referrals to Programs
PCMH makes every effort to facilitate easy and timely access to services. Clinical, financial and psychosocial information is required in order to establish eligibility for programs.
