Employment Services
PCMH operates a full-spectrum employment program aimed at reintegrating consumers into the world of work. The program promotes self-sufficiency and challenges society's stigmatization of persons with mental illness. PCMH offers didactic training services, placement and support services, and competitive employment opportunities. Clients who have limited work histories resulting from their mental illness find the supports they need to succeed in the workplace. Services are funded through contracts with the New York State Department of Vocational and Educational Services for Individuals with Disabilities (VESID), State Office of Mental Health (SOMH) and New York City Department of Health and Mental Health (DOHMH).

Blended Case Management
Offers care coordination and support services to seriously and persistently mentally ill adults and seriously emotionally disturbed children. The program is designed to assist individuals to remain connected to the mental health system, to coordinate the mental health services they receive and to overcome barriers to accessing services that often accompany chronic mental illness. Case managers assertively engage clients and families and provide linkages to programs that include clinical, medical, educational, self help, social remedial, supportive and rehabilitative services. Case managers provide advocacy around entitlements; monitor medication compliance as needed; identify and use existing community resources, such as vocational training and recreational services.
Project US
PCMH sponsors Project Us, a unique club, organized and operated by consumers of mental health services. This cost-efficient, consumer run program stresses self help and empowerment. The club operates in the evenings and weekends so that members can participate in social and recreational activities. Members select from a full recreational program those activities and groups that interest them.
Referrals to Programs
PCMH makes every effort to facilitate easy and timely access to services. Clinical, financial and psychosocial information is required in order to establish eligibility for programs.
