Care management
CARE MANAGEMENT is a program which enables individuals with significant medical and other needs to remain in a home or community setting with a high quality of life. This is a personalized program that provides assistance with conditions related to aging, physical illness, disability, mental illness, developmental disabilities, end of life issues, family/caregiver challenges.
The services offered include comprehensive needs assessments; care planning; care plan implementation and monitoring; interviewing, training and coordination of home care workers; counseling and family interventions; crisis intervention; home, hospital or nursing home visits; assisted living or nursing home placements; and care coordination with other health care providers
